In April 2010 a new system was introduced to simplify the social housing application process. Housing NSW and community housing providers worked together to set up one application system called Housing Pathways.
In order for you to apply for re-housing you will need to fill out the following forms, Application for Housing Assistance, Transfer Supplement Community Housing Tenants and the Evidence Requirements sheet. Please forward these forms to Affordable Community Housing Limited, PO 7096, Silverwater BC NSW 1811.
